Signed and with the following text: "El Ayuntamiento Constl. de Sarriá, provincia de Barcelona, en atención a los beneficios qe. ha dispensado a este pueblo el Ilmo. Sr. D. José María Cornet y Mas acordó por voto unánime en sesion del 22 de Abril del actual año dar el nombre de Cornet y Mas a una de las principales calles del mismo modo como prueba de su profundo agradecimiento e interpretando así los deseos de toda la poblacion. Y en virtud del calendado acuerdo se expide el presente documento en Sarrià a 14 de Mayo de 1897. El Alcalde Prete. y P. A. del Ayunto.El Secretario".
